Activity 3:
Designing a Wrap Team based on Strengths, Needs,
Natural Supports and available/needed Resources
|
Use page 1 of the CTPF to map the potential team membership of the youth
from role-play or actual youth/family you are working with per activities
4.1. and 4.2. See Handout 4.6 for CTPF.
Consider the following as you do this activity:
- Core team members are those who have major responsibility, daily
or almost daily contact with the student or control over major decision-making
regarding the student.
- Extended team members may be critical to effective interventions
(i.e. a friend or a coach) but may not be a key decision-maker about
major decisions such as where the student lives or attends school
etc.
- Consider natural support persons, persons representing strengths
of the youth and people representing resources/skills needed to meet
needs identified by the youth/family.
- Team membership should reflect culture, values, life experiences
of the youth/family.
- Team membership is key to the youth/family feeling as if it is their
team and their plan instead of feeling as if they are attending a
meeting some system (i.e. school) is having about them.
