Documenting Team Structure and Life Domain Profile
from Information Obtained During Initial Conversations


Review and discuss the information you have gathered during initial conversations and documented on pages 1-3 of the CTPF (see activities 4.2 and 4.3).

Review this information with a classmate or a colleague who is also involved with increasing their skills/knowledge about wraparound.

Discuss team composition by role, strengths, perspectives of core team members, and potential needs statements to review with the team.

Identify similarities and perspectives of team members that you (as team facilitator) would need to be aware of when guiding the team to consensus on needs and brainstorming strategies.
